How to get it
1
Make an appointment with your family doctor or nurse practitioner to discuss the breast cancer in your family.
2
If you don’t have a family doctor, the Primary Care Access Point (GAP) can help you get an appointment. You fill out a request online, or call 811 and choose option 3 if you need help with it, and someone calls you back.
3
Bring a note of who had breast cancer and how old they were when it was diagnosed.
4
Depending on your risk, you may be prescribed a mammogram or other tests and given a follow-up plan.
